- Fever over 100 degrees, two or more episodes of vomiting
- Fractured/lost adult tooth, toothache
- Lacerations, sprains, burns, visible bumps/bruises
- Eye/head injury, earache
- Allergic reaction, rashes, hives
- Any medication administered to student

- Single episode of vomiting or headache.
- Mouth ulcer or lost baby tooth
- Small cuts, splinters (unless cannot be removed)
- Nosebleed, lip balm, band aids, masks
- Insect bites other than ticks; bee sting without allergic reaction
